Complaint or Concern Report Form

At Rockvale Youth Football & Cheer (RYFC), the safety, well-being, and positive development of every child is our top priority. To support that commitment, we’ve created this confidential Complaint or Concern Report Form.

This form is designed to give parents, volunteers, and community members a clear path for reporting:

Inappropriate behavior by coaches, referees, volunteers, or parents
Player or cheerleader conduct that raises safety or sportsmanship concerns
Unsafe conditions or irregularities within our facilities, practices, or games
Any situation that may be detrimental to a child’s well-being or the integrity of our program

How It Works:

All submissions are kept confidential.
You may choose to remain anonymous.
However, providing your contact information will help us investigate and follow up effectively.
Each report is reviewed by our Executive Director and/or relevant Board members in a timely and discreet manner.

We encourage all members of Rocket Nation to speak up if something doesn’t seem right. Together, we can maintain a safe, respectful, and encouraging environment for all our athletes and families.
